CAS offers its members the benefit of collecting research data from other CAS members in the form of surveys. Surveys are a great way for associations and researchers to collect key data from the anesthesiology demographic.

Please fill out the form fields below and return a signed copy of a the “Member Survey Distribution Agreement,” as soon as possible. Survey fees and email distribution details are outlined in the agreement.  

Note that CAS directly distributes all surveys and reviews all applications before distribution. Please allow for up to 30 days for the CAS office to review your survey.

• Member Survey Application
• CAS Member Survey Distribution Agreement
• Schedule A: Study protocol of your survey
• Schedule B: PDF of the survey questions
• Schedule C: Link to the survey
• Cover letter (the text our members will read)
• Ethics approval

SURVEY DISTRIBUTION

CAS will send two (2) emails on behalf of the Survey author/institution:

• Initial survey request
• One (1) reminder email

Survey Fees

• Initial survey set-up, subsequent work requested (reminder, etc.) - $1000.00
• Email distribution - $0.10 per email sent

Please note that survey applications must be submitted 4-6 weeks prior to the desired distribution date.
