Ambulatory total hip and knee arthroplasty: a literature review and perioperative considerations

Ambulatory total hip and knee arthroplasty: a literature review and perioperative considerations
Total joint arthroplasty (TJA), particularly for the hip and knee, is one of the most commonly performed surgical procedures. The advancement/evolution of surgical and anesthesia techniques have allowed TJA to be performed on an ambulatory/same-day discharge basis. In this Continuing Professional Development module, we synthesize the perioperative evidence that may aid the development of successful ambulatory TJA pathways....

Tranexamic acid: current use in obstetrics, major orthopedic, and trauma surgery

Tranexamic acid: current use in obstetrics, major orthopedic, and trauma surgery
Tranexamic acid is an efficacious and safe pharmacological-based blood conservation technique in the management of clinically significant hemorrhage, and its use is likely to continue to rise with endorsement by various clinical guidelines and healthcare organizations. All anesthesiologists should have a good understanding of the pharmacotherapeutic properties and perioperative role of tranexamic acid therapy both inside and outside of the operating room. In this Continuing Professional Development module, we review the practical pharmacology of tranexamic acid and its clinical use in trauma, obstetrics, and major orthopedic surgery....

An updated introduction to electroencephalogram-based brain monitoring during intended general anesthesia

An updated introduction to electroencephalogram-based brain monitoring during intended general anesthesia
Given that consciousness is dependent on neuronal activity, and that general anesthetics have their disruptive effect at the neuronal level, a measure of neuronal activity when responsiveness and memory are disrupted should be of deep interest to every anesthesiologist. The electroencephalogram (EEG) is an easily obtained measure of neural activity, quantifying non-invasively from the scalp the summed, synchronous electrical activity of millions of neurons in the cerebral cortex.

Synopsis of the point-of-care ultrasound assessment for perioperative emergencies
This module introduces the concept of a point-of-care ultrasound (POCUS) examination for perioperative clinicians. A focused cardiac examination of ventricular filling and function is presented. An examination of the inferior vena cava is also reviewed as a tool to assess volume status. Finally, a brief examination of the lung and pleura is explored to aid the clinician in situations of patient hypoxia and difficult ventilation....

Anesthetic implications of recreational drug use

Anesthetic implications of recreational drug use
As the use of recreational drugs increases, the likelihood of an anesthesiologist perioperatively encountering patients using or addicted to these drugs will also increase.

Addicted patients may present for anesthetic care in a variety of circumstances in everyday elective surgeries or in acute or life-saving situations, such as emergency Cesarean delivery or trauma surgery. Therefore, it is important for anesthesiologists to know about the most common illicit drugs being used, their clinical presentation and side effects, and the anesthetic options that are beneficial or detrimental to these patients....

Managing the perioperative patient on direct oral anticoagulants

Managing the perioperative patient on direct oral anticoagulants
Patients are increasingly treated with direct oral anticoagulants (DOACs) for the prevention of stroke due to non-valvular atrial fibrillation and for the treatment of venous thromboembolism. When these patients present for urgent or emergent surgical procedures, they present a challenge to the anesthesiologist who must manage perioperative risk due to anticoagulation. The purpose of this module is to review the literature surrounding the perioperative management of DOACs. Timing, laboratory monitoring, and availability of reversal agents are important considerations to optimize patients being treated with DOACs who require emergent surgery....
